- If you received a summons for jury duty, please read & view it carefully, as it is two sided. The top portion includes information on the date and time of your jury trial to which you were summoned for, along with the juror questionnaire that needs to be filled out entirely. The back side includes the legal exemptions allowed per the Texas Government Code. Should you qualify for one of the allowed exemptions, please fill out the jury card questionnaire entirely and place it in an envelope with a stamp and return it to the address listed on the sticker provided.
